Description

Case Manager

Location: Fort Wayne, Indiana
Employment Type: Traveler - Backfill
Contract Length: 13 weeks
Shift: 8-hour day shifts
Hours per Week: 40


Job Description

  • Collaborate with the Care Team to complete comprehensive patient assessments focused on patient status to drive discharge.
  • Drive stay progression and complete discharge planning to expedite patient transitions to the next level of care.
  • Perform comprehensive and accurate documentation using Epic.
  • Primarily assigned to one unit but may float to other units/floors performing inpatient case management duties.
  • Units range from 20-36 beds, with one or two RN Case Managers per unit.
  • Participate in weekend rotation; call requirements specified per assignment.

Required Qualifications

  • Minimum 2 years of inpatient RN Case Manager experience.
  • Valid Indiana RN license or compact RN license.
  • Preferred certification in Case Management.
  • Ability to work independently and collaboratively with the care team.
  • Strong documentation skills with Epic.

Orientation & Training

  • General orientation (6 hours).
  • Epic training (4 hours).
  • Role-specific training with peer (8-16 hours).
  • Limited assignment with peer oversight (8-16 hours).

Work Environment & Dress Code

  • Business casual attire with white lab coat (no jeans).
  • Close-toed shoes required.
  • Option to wear eggplant-colored scrubs (top and bottom).

Equipment Used

  • Epic electronic documentation system.
  • Cell phone for communication.

Additional Notes

  • Candidates should communicate any required time off during the assignment.
  • Floating involves performing the same inpatient case management duties regardless of unit.
