Description

Transitional Care Management RN

Shift: 8:00 AM – 4:00 PM
Weekends: Occasional
Program: New Quality Department transitional care program
EHR: Epic
Role: Coordinates patient care transitions after discharge from inpatient, ED, SNF, or other care settings
Skills: Post-discharge outreach, medication reconciliation, discharge instruction review, symptom assessment, barrier identification, TCM documentation
Care Coordination: Schedules follow-ups, monitors high-risk patients, escalates concerns, and supports referrals to home care, behavioral health, palliative care, pharmacy, or community resources
United Staffing Solutions Job ID #3330402. Pay package is based on 12 hour shifts and 36 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
