Description
Job Opportunity: RN Case Manager
Location: Cortlandt Manor, New York
Employment Type: Long Term
Contract Length: 13 weeks (with likely extension)
Shift: Day shift, primarily 4/10's (7am-5pm or 8am-6pm) with every other Sunday and 2 weekends per month; some flexibility for 5/8's (8am-4pm or 9am-5pm) plus 2 weekends per month
Expected Hours: 40 hours per week
Job Description
- Role: BSN, RN Case Manager
- Recent experience required: Minimum 3 years working with DCP (Discharge Care Planning or similar case management process)
Required Qualifications
- Valid Registered Nurse (RN) license in New York
- Bachelor of Science in Nursing (BSN) degree
- Minimum 3 years of case management experience, including experience within the last year
- Minimum 3 years of experience with Discharge Care Planning (DCP)
Additional Information
- Start date: June 7, 2026
- Schedule includes some weekend work (2 weekends per month) and every other Sunday
This position is suitable for experienced RN Case Managers seeking a long-term contract with a structured day shift schedule and flexibility in hours.
