Description
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. As a key member of the care delivery team, you will play a vital role in supporting patients on their health journey—right where they feel most comfortable: at home.
Position Summary:
The Registered Nurse – Home Health provides personalized, high-quality nursing care to patients in their homes. This role allows for autonomy in your practice, offers flexible scheduling, and provides the opportunity to build meaningful relationships through 1:1 patient care. You’ll work closely with a supportive interdisciplinary team to coordinate and deliver the right care at the right time.
Key Responsibilities:
- Perform initial and ongoing assessments of patients’ physical, psychological, social, and environmental needs.
- Develop and implement individualized care plans in coordination with patients, families, and the interdisciplinary team.
- Reassess patients regularly and adjust care plans as needed based on changes in condition or circumstances.
- Communicate significant findings and changes in patient status to supervisors, physicians, and other care team members.
- Maintain accurate clinical documentation in compliance with regulatory and organizational standards.
- Uphold patient confidentiality and privacy in accordance with HIPAA and other regulations.
- Collaborate with other clinicians to ensure coordinated, efficient, and high-quality care.
- Current, unrestricted RN license in the state of practice
- Minimum of 1 year of nursing experience in a clinical care setting
- Valid driver’s license, reliable transportation, and active auto insurance
- Willingness and ability to travel within assigned service area (up to 50%)
- Associate’s or Bachelor’s Degree in Nursing (preferred)
- Prior experience in Home Health or Hospice (preferred)
- Familiarity with HomeCare HomeBase or similar EMR systems (preferred)
