Job Description

American Traveler is seeking an experienced Physical Therapist for a Home Health outpatient position requiring at least one year of recent specialty experience and an active state license.

Responsibilities

• Position is based in a home health outpatient setting
• Assignments are 13 weeks long with a day shift schedule, Monday through Friday from 8:00 AM to 4:30 PM
• Patients are seen in their homes in the local community
• Outpatient physical therapy services will be provided
• Provide patient assessments and develop individualized treatment plans for patients at home
• Complete daily documentation in accordance with agency standards
• Maintain effective communication with the interdisciplinary care team
• Requires travel to patients' homes; reliable transportation is necessary
• Applicants must have proof of COVID and flu vaccination, if required

Requirements

• Active Physical Therapy state license (VA or Compact)
• Minimum of one year of recent physical therapy experience in a home health or outpatient setting
• All certifications must be from AHA and valid; no certifications may expire within the first 30 days of assignment
• Must not reside within 65 miles of the worksite; local candidates are not eligible
• No gaps in work history of 90 days or more within the past 12 months
• U.S. Social Security Number required for consideration
