Description

American Traveler is hiring a Home Health Occupational Therapist with a minimum of 3 years of experience for a 13-week assignment in Oregon.

Details

• Home health setting with patient care provided in patients' homes
• Day shift, 8:00 AM – 4:30 PM
• 40 hours per week
• 13-week contract

Requirements

• Active Oregon OT license required (license required if applicable)
• Minimum 3 years of OT experience required
• Current BLS certification required
• Valid driver's license and proof of auto insurance required
• Must have own vehicle for patient home visits
• Graduate of an accredited occupational therapy curriculum
• Eligible for or registered with the National Registration Examination of the American Occupational Therapy Association

Additional Information

• Evaluate patients' functional status and implement physician-directed plans of care in a home health and hospice setting
• Perform assessments and reassessments using objective tests and measurements
• Collaborate as part of an interdisciplinary team through communication and team meetings to coordinate patient care
• Provide education and direction to patients and coworkers as appropriate
• Complete accurate, concise documentation of all communications, coordination, and treatment activity
• Work under the supervision of the Home Health and Hospice Manager
• CRASE training may be required if the assignment extends beyond 6 months
• Registration with the American Occupational Therapy Association preferred
• Continuing education relevant to physical disability, rehabilitation, mental health, elderly population, long-term care, and home health preferred
• DOB and SSN may be required for consideration
