Description

American Traveler is seeking a Home Health Physical Therapist with a minimum of 1 year of recent experience, OASIS proficiency, and HCHB EMR experience for a 13-week assignment.

Details

Home health setting serving a geriatric patient population
Average caseload of 5–7 patients per day (to be confirmed during interview)
Territory covers up to a 60-mile radius from the hub/office; flexibility with territory is expected
Day shift schedule, 5x8-hour shifts (8:00–17:00)
On-call rotation required approximately every 3rd to 4th weekend
EMR: HCHB (Homecare Homebase) experience required
iPad provided for documentation; personal cell phone not provided
Mileage reimbursed at the IRS standard rate

Requirements

Minimum 1 year of recent home health physical therapy experience
OASIS experience required for Start of Care (SOC) visits
Proficiency in HCHB (Homecare Homebase) required
Current BLS certification required
Active driver's license and personal transportation required
PT license in hand required for consideration (license required if applicable)
RTO must be submitted at the time of application

Additional Information

Responsibilities include physical therapy evaluation and treatment of geriatric home health patients with timely documentation and communication required
Orientation is held on Mondays; required modules are completed during orientation
Scrubs of any color are acceptable; jeans are not permitted
Local candidates are accepted at the travel rate
First-time travelers are accepted
Travel pairs are accepted; however, each traveler will be assigned their own territory and must have their own transportation
Former permanent employees must have been separated for a minimum of 6 months and receive HR approval before returning as a traveler
