Description

American Traveler is seeking a Physical Therapist (PT) for a 13-week Home Health assignment, requiring at least 1 year of experience and active BLS certification.

Details

• Home health care setting serving most of Tehama County
• Average daily census of 78 patients, expected to grow with PT availability
• Patient population includes orthopedic/joint replacement and geriatric patients with multiple comorbidities such as CHF and COPD
• PTs perform Start of Care (SOC) visits, particularly for orthopedic patients when nursing is not ordered
• 5 patient visits per day
• EMR: HCHB (HomeCare HomeBase); staff are provided a tablet for charting
• Cerner also noted as a charting system for this position
• 8-hour day shifts, Monday–Friday; no weekend or call requirements
• Holidays off
• Anticipated daily mileage of approximately 40 miles, expected to decrease as therapy staff grows and patient loads are redistributed by area

Requirements

• Minimum 1 year of physical therapy experience required; candidates with less than 2 years of experience must have Home Health experience
• Home Health experience required
• Active PT license required; pending licenses will not be accepted — license required if applicable
• Active BLS (AHA) certification required; pending certifications not accepted
• Driver's license required for consideration
• 2 professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, both including dates of employment and eligibility for rehire

Additional Information

• Responsibilities include home health evaluations, SOC visits, and ongoing treatment across a geographically distributed service area
• Support structure includes an intake RN and manager in the office, as well as nurse aides
• 12-hour on-site orientation provided
• Candidates who have been directly employed (full-time, part-time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity, within the past year are not eligible; candidates employed PRN through outside firms will be considered
