Description

American Traveler is hiring an experienced RN for a Care Coordinator position specializing in heart transplant patient management, requiring a CA RN license and cardiac nursing experience.

Details

  • Ambulatory care setting within a large academic health system
  • Care Coordinator unit focused on cardiac and transplant patient populations
  • Primary patient population includes heart transplant recipients and patients with heart failure
  • Day shift, 4x10-hour shifts, 7:30 AM – 6:00 PM
  • Floating to other facilities within the health system is required

Requirements

  • Active CA RN license required
  • Current AHA BLS certification required
  • Minimum 1 year of ambulatory care nursing experience required
  • Cardiac nursing experience with management of heart failure and/or heart transplant recipients required
  • Experience with Phoenix EMR is preferred
  • Working knowledge of immunosuppressive therapies preferred
  • Experience managing other organ transplant recipients preferred

Additional Information

  • Responsibilities include coordination of care for heart transplant patients, encompassing immunosuppression monitoring, prophylactic antibiotic management, and monitoring for organ rejection and infection
  • Position is based at the Parnassus campus of a major academic medical center in San Francisco
