Description
American Traveler is hiring an experienced RN for a Care Coordinator position specializing in heart transplant patient management, requiring a CA RN license and cardiac nursing experience.
Details
- Ambulatory care setting within a large academic health system
- Care Coordinator unit focused on cardiac and transplant patient populations
- Primary patient population includes heart transplant recipients and patients with heart failure
- Day shift, 4x10-hour shifts, 7:30 AM – 6:00 PM
- Floating to other facilities within the health system is required
Requirements
- Active CA RN license required
- Current AHA BLS certification required
- Minimum 1 year of ambulatory care nursing experience required
- Cardiac nursing experience with management of heart failure and/or heart transplant recipients required
- Experience with Phoenix EMR is preferred
- Working knowledge of immunosuppressive therapies preferred
- Experience managing other organ transplant recipients preferred
Additional Information
- Responsibilities include coordination of care for heart transplant patients, encompassing immunosuppression monitoring, prophylactic antibiotic management, and monitoring for organ rejection and infection
- Position is based at the Parnassus campus of a major academic medical center in San Francisco
