Description
American Traveler is seeking an experienced RN Case Manager for a supervisory/manager role in a hospital Case Management unit, requiring at least 5 years of experience with 3–5 years in a leadership capacity.
Details
- Hospital-based Case Management unit serving a diverse inpatient population including NICU, pediatric, ER, Med/Surg, Telemetry, Oncology, and ICU (no L&D)
- Manager-level position responsible for supervising the RN/CM unit
- Caseload ranges from approximately 15 to 25–30 patients depending on staffing levels
- 8-hour day shifts, 40 hours per week
- Floating throughout the hospital as needed
- EMR: Epic (experience preferred)
Requirements
- Active WA state RN license or compact license (currently in hand or pending with proof of application)
- BSN required
- Current BLS certification required
- Minimum 5 years of RN Case Management experience required, with at least 3–5 years of supervisory or management experience
- Inpatient clinical background strongly preferred
- Proficiency in case management and discharge planning, including discharging homeless patients and transitioning patients to SNF, LTAC, IP rehab, assisted living, and shelter settings
- Experience managing Medicare/Medicaid patient populations
- Strong clinical assessment skills
- At least one supervisor-level reference required for consideration (Charge Nurse, Manager, Director, or above); a second reference will be required prior to start
- First-time travelers will not be considered for this position
Additional Information
- Responsibilities include supervising the RN/CM unit, overseeing discharge planning, and coordinating transitions to the appropriate next level of care
- Partnered with a dedicated Case Management Social Worker; support team also includes Case Management Assistants and a CM Manager available daily
- Orientation includes general hospital onboarding (NEO) followed by 2 weeks of department-specific training
- Any solid color scrubs or business casual attire are acceptable
- Small hospital with a close-knit team; candidates who are flexible and collaborative team players are encouraged to apply
- Free parking is available on-site
