Description
American Traveler is hiring an experienced RN for a Hospice Case Manager position in Loudoun County, VA, requiring a minimum of 1 year of recent hospice experience and a VA or compact RN license in hand.
Details
- Home-based hospice setting serving Loudoun County, VA
- Specialty focus on geriatric and end-of-life care
- Caseload of approximately 5–7 patients per day
- Day shift, 5x8-hour shifts (8:00 AM – 5:00 PM)
- No on-call responsibilities
- Territory covers up to a 60-mile radius from the hub/office; flexibility with territory is expected
- EMR: Netsmart
- iPad provided for use in the field
Requirements
- Active VA or compact RN license currently in hand
- Current BLS certification required
- Minimum 1 year of recent hospice nursing experience required
Additional Information
- Responsibilities include case management for hospice patients in a home-based setting
- Timely documentation and communication are essential to this role
- Mileage reimbursed at the IRS standard rate
- Scrubs required in any color; jeans are not permitted
- Local candidates are accepted and eligible for the travel rate
- Two professional references from a supervisor within the past 2 years are required for consideration
- All time-off requests must be submitted at the time of initial consideration
