Description
American Traveler is seeking an experienced Physical Therapist for a Home Health position covering Snohomish and Skagit County, requiring a minimum of 1 year of recent experience and proficiency with HCHB and OASIS documentation.
Details
- Home health setting serving a geriatric patient population across Snohomish and Skagit County
- Territory coverage of up to a 60-mile radius from the hub/office; travelers are expected to be flexible with territory
- Caseload of approximately 5–7 patients per day with a minimum of 6 visits daily
- Minimum productivity expectation of 30 points per week
- Schedule is Monday–Friday, 8-hour day shifts (8:00 AM–5:00 PM) with no weekend or on-call requirements
- Clinician is expected to visit the main office at least once a week or every two weeks for team meetings, supply pickup, and admission packets
- EMR: HCHB (experience required)
- OASIS experience required
Requirements
- Minimum 1 year of recent Physical Therapy experience in home health
- Active PT license required if applicable
- Current BLS certification required
- Proficiency with HCHB EMR required
- OASIS documentation experience required
- Must be able to travel within a 60-mile service territory
- Strong time management skills with the ability to meet productivity and documentation standards
- First-time travelers will not be considered for this position
Additional Information
- Responsibilities include conducting home visits, completing timely documentation, and maintaining consistent communication with the care team
- An iPad is provided for use in the field; personal cell phone is not required
- Mileage is reimbursed at the IRS standard rate
- Scrubs in any color are required; jeans are not permitted
- Local candidates are accepted and eligible for the travel pay rate
- All time-off requests should be disclosed prior to or at the time of initial consideration
- 2 professional supervisory references from within the past 2 years are required for consideration
- A full 7-year work history with all gaps explained is required for consideration
