Description
American Traveler is hiring a travel RN - Case Manager for an acute care case management position requiring at least 1 year of case management experience and an active UT or compact RN license.
Details
- Acute care hospital setting, Case Management unit
- Coordinates case management and discharge planning across the care continuum, from patient admission through post-discharge
- Caseload includes patients and families requiring care coordination with payers, vendors, and multidisciplinary team members
- Day shift schedule: 5x8-hour shifts, 8:00 AM – 4:30 PM
- 13-week contract
Requirements
- Active UT RN license or compact RN license currently in hand
- Current BLS certification required
- Minimum 1 year of case management experience in an acute care setting required
Additional Information
- Responsibilities include monitoring patient progress toward care goals, resolving barriers to effective care, and coordinating discharge planning alongside internal team members
- May be required to access and administer medications within scope of practice and per state law
- Candidates must reside at least 100 miles from the facility to be eligible — candidates living within the state of Utah will not be considered
