Description

American Traveler is hiring a travel RN - Case Manager for an acute care case management position requiring at least 1 year of case management experience and an active UT or compact RN license.

Details

  • Acute care hospital setting, Case Management unit
  • Coordinates case management and discharge planning across the care continuum, from patient admission through post-discharge
  • Caseload includes patients and families requiring care coordination with payers, vendors, and multidisciplinary team members
  • Day shift schedule: 5x8-hour shifts, 8:00 AM – 4:30 PM
  • 13-week contract

Requirements

  • Active UT RN license or compact RN license currently in hand
  • Current BLS certification required
  • Minimum 1 year of case management experience in an acute care setting required

Additional Information

  • Responsibilities include monitoring patient progress toward care goals, resolving barriers to effective care, and coordinating discharge planning alongside internal team members
  • May be required to access and administer medications within scope of practice and per state law
  • Candidates must reside at least 100 miles from the facility to be eligible — candidates living within the state of Utah will not be considered
