Description
American Traveler is seeking a licensed Physical Therapist with at least 1 year of certified home health experience for a 13-week home health float assignment in the North Shore region of Massachusetts.
Details
- Home health setting serving patients in their place of residence across the Lynn, Nahant, and Saugus areas of Massachusetts
- Float position covering the North Region service area; ability to travel to cases as assigned is required
- Patient population ranges from birth to 100+ years with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
- Shift hours are 8:00 AM – 4:30 PM
- Weekend rotation required approximately once every 6 weeks
- EMR: Epic (charting experience required)
Requirements
- Active Massachusetts Physical Therapist license required; MA Conditional Licenses are not accepted
- Minimum 1 year of experience in a certified home health agency required — hospice, private duty, or case management experience alone does not satisfy this requirement
- Minimum 1 year of case management experience required
- AHA certifications only accepted
- Flu vaccine required; no religious or medical exemptions are permitted
- License required if applicable
- 2 professional references required for consideration
Additional Information
- Primary responsibilities include evaluating and treating patients in their home, developing and implementing individualized therapy plans, and collaborating with patients, families, physicians, and allied health professionals to coordinate care
- Timely and thorough electronic documentation and interdisciplinary communication are core expectations of this role
- Timekeeping system: UKG/Kronos
- Candidates with a permanent residence within 60 miles of Massachusetts General Hospital in Boston are considered local and are eligible for this position
- No validated parking is available at Boston locations; public parking lots are available at daily rates, and public transportation is also an option — parking costs may apply at other locations as well
- Any RTO (requested time off) should be disclosed at the time of consideration
