Description

American Traveler is hiring a travel RN for a Float Home Health position in the North Region (Chelsea, East Boston, Revere, and Winthrop areas), requiring at least 1 year of certified home health agency experience and an active MA RN license.

Details

• Home health care setting serving patients across the Chelsea, East Boston, Revere, and Winthrop areas
• Float role requiring travel to patient cases as assigned across the North Region
• Day shift schedule: 8:00 AM – 4:30 PM
• Weekend rotation required approximately once every 6 weeks
• 13-week contract
• Epic used for electronic charting

Requirements

• Active MA RN license required — no pending or conditional licenses accepted
• All certifications must be from the American Heart Association (AHA)
• Minimum 1 year of experience in a certified home health agency required — hospice, private duty, or standalone case management roles do not fulfill this requirement
• Minimum 1 year of case management experience required
• Ability to travel independently to patient cases as assigned
• 2 professional references required for consideration
• Flu vaccine required; no religious or medical exemptions accepted

Additional Information

• Responsibilities include assessing patient conditions, developing and implementing care plans, and providing education and support to patients and families
• Performs admission processes and case management duties in accordance with home care policies and standards of practice
• Requires thorough and timely electronic documentation and interdisciplinary communication
• Care is delivered in alignment with community health practice philosophy and organizational policy
• Timekeeping is managed through UKG/Kronos
• No validated parking available at Boston-area locations; public parking costs approximately $17–$22 per day — public transportation is recommended
• Local candidates residing within 60 miles of the Boston facility are accepted at a reduced rate
• Any requested time off (RTO) must be disclosed at the time of consideration
