Description
American Traveler is hiring a Home Health RN Float for a 13-week contract requiring certified home care and case management experience with active MA licensure.
Details
- Home Health float position covering North Region Scheduling area including Chelsea, East Boston, Revere, and Winthrop
- 13-week contract assignment
- Day shift schedule from 08:00-16:30
- Weekend rotation once every 6 weeks
- Requires travel to patient homes as assigned
- Uses Epic for charting
Requirements
- Active MA RN license required (no conditional or pending licenses accepted)
- AHA-issued BLS certification required
- Minimum 1 year of experience specifically within a certified home health agency (hospice, private care, or standalone case management roles do not qualify)
- Minimum 1 year of case management experience
- Flu vaccine required with no religious or medical exemptions allowed
- Primary source license verification required within 30 days of start and upon renewal
Additional Information
- Assess patient conditions, implement care plans, and provide ongoing education and support to patients and families
- Perform admission processes and case management responsibilities per organizational policies and standards
- Maintain thorough and timely electronic documentation and interdisciplinary communication
- Deliver care in accordance with community health practice standards
- Local candidates must reside within 60 miles of Massachusetts General Hospital in Boston
- Parking not validated at Boston locations; public transportation or paid parking required
