Description
American Traveler is hiring a travel RN Case Manager for a Utilization Review role in Paramount, CA, requiring a CA RN license and a minimum of 5 years of RN Case Management experience in an LTAC or STACH setting.
Details
- Long-term acute care (LTAC) or short-term acute care hospital (STACH) setting
- Unit: Utilization Review / Case Management
- Day shift, Monday–Friday, 8:00 AM–4:30 PM
- Weekends on request only
- 13-week contract
Requirements
- Active California RN license required
- BLS certification required (American Heart Association only; RQI certifications not accepted)
- Minimum 5 years of recent RN Case Management experience required
- Experience in an LTAC or STACH setting required
- Experience with MCG guidelines and discharge planning across acute care settings required
- Strong interdisciplinary team (IDT) collaboration skills required
- No gaps in work history of 90 days or more within the past 12 months
- SSN and date of birth required for consideration
- Candidates must reside more than 50 miles from the facility to qualify for the travel pay rate
Additional Information
- Coordinates care management and ensures optimal utilization of resources, service delivery, and compliance with external review agencies
- Performs comprehensive patient assessments, care planning, and plan implementation to address individual patient needs
- Integrates case management, utilization review, and discharge planning functions to promote continuity of care and cost-effectiveness
- Serves as a patient advocate by addressing holistic patient needs, managing resources, and coordinating discharge planning
- Collaborates closely with the interdisciplinary team throughout the patient care continuum
- Candidates who have previously worked at a Kindred Hospital facility (permanent or contract) should be prepared to disclose the facility name, dates worked, and reporting manager
- Any requested time off during the contract term must be disclosed upfront
