Requirements:
Active VT &amp; NH RN License or Compact RN License
BLS Certification
Minimum 3 years of recent Home Health experience
Graduate of an accredited nursing program

Responsibilities:
Patient assessments and care planning
Case management and interdisciplinary coordination
Patient/caregiver education
Clinical documentation and ongoing evaluations

Coverage area includes multiple locations throughout Vermont and New Hampshire. Mileage reimbursement provided.
