Description
American Traveler is seeking an experienced RN Care Manager for a 13-week hospital-based assignment in York, Maine, requiring utilization management and discharge planning experience.
Details
- Hospital-based care management role working house-wide across an inpatient population in Southern Maine
- Variable shift schedule: either 8:30 AM–4:30 PM (8-hour shifts) or 7:30 AM–5:00 PM (four 10-hour shifts per week)
- No on-call requirements
- Weekend and holiday rotation required
- 40 hours per week
Requirements
- Active ME or compact RN license currently in hand
- Current BLS/BCLS certification required; CCM or ACM certification preferred
- Minimum 3 years of clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review
- Utilization review and discharge planning experience required
- Associate or Bachelor's degree in Nursing required
Additional Information
- Coordinate concurrent utilization management activities across the hospital, serving as the primary link between patients, the healthcare team, and payors throughout the hospital stay
- Conduct patient needs assessments, identify transition resources, and secure necessary services for next-level care
- Collaborate with and lead an interdisciplinary team during daily care rounds, care conferences, and care planning
- Work with community organizations to arrange durable medical equipment, insurance authorizations, and transportation
- Apply knowledge of hospital reimbursement methods and quality measures relevant to care management
- Strong written, verbal, and electronic communication skills required, including proficiency with hospital software applications
