Description

American Traveler is seeking an experienced RN Case Manager for a 13-week travel contract in a hospital Case Management setting, requiring a KS or compact RN license and at least 1 year of experience.

• Day shift schedule: 8:00 AM – 4:30 PM
• 13-week contract

Requirements

• Active KS or compact RN license currently in hand
• BLS certification required (AHA accepted)
• Minimum 1 year of RN case management experience
• Valid U.S. Social Security Number and U.S. home address required
• Two professional references required for consideration
• Copies of certifications, license verification/Nursys, and Driver's License required for consideration
• Full SSN and full DOB required for consideration

Additional Information

• Coordinates the interdisciplinary plan of care for patients from admission through discharge
• Integrates patient and family preferences and values into the care plan
• Monitors patients' medical necessity and level of care through assessments, ongoing evaluations, and patient records
• Acts as a liaison between the patient and post-acute services, and collaborates with payers for benefit optimization
• Advocates for effective utilization of clinical resources and removal of barriers to care
• MOAB De-Escalation Training may be required prior to start
• Local candidates are accepted; however, candidates residing within the facility's radius may be eligible at a reduced pay rate only
