Description
American Traveler is seeking a licensed Physical Therapist with home health experience for a 20-week home health contract in Tampa, FL, serving a diverse patient population across Medicare, Medicare Advantage, and commercial insurance plans.
Details
- Home health setting providing skilled visits to patients in their place of residence
- Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL patients
- Skilled home care services provided under Medicare, Medicare Advantage, and commercial insurance
- 6–8 patient visits per day, with a minimum of 6 points per day (may increase at manager's discretion)
- Day shift schedule, 5x8-hour shifts (8:00–17:00), self-scheduled based on HomeCare office needs
- 1–2 weekend shifts required per contract
- Equipment provided includes laptop, MiFi for internet access, and portable printer
- Documentation systems used: BayCare Forms, BayCare Scheduling, and ACE Timesheet
Requirements
- Minimum 1 year of physical therapy experience required; home health experience required
- Current BLS certification required
- State PT license required (license required if applicable)
- Valid driver's license and personal vehicle required to conduct home visits
- Proficiency in assessment, cardiopulmonary/circulatory care, documentation, and billing
- Experience with adaptive equipment including gait belts, rolling walkers, sit-to-stand lifts, sliding boards, soft/hard collars, standard walkers, supplemental oxygen, upper extremity prosthetics, and wheelchair assessment
- Experience in ortho, neuro, home exercise program development, and patient/family education
- Knowledge of medication management, signs and symptoms of physical/sexual/psychological abuse, and safety and injury prevention
- OASIS training preferred
- COVID vaccination or approved exemption form required
- Flu shot or signed declination required; mandatory mask wearing at all times while on assignment
Additional Information
- Responsibilities include skilled home visits, patient/family education, home exercise program development, and thorough documentation
- 2 professional references from a supervisor within the past 2 years required for consideration
- Scrubs must be any solid color
- Candidates must wear a mask at all times while working, regardless of flu shot status
