Description

American Traveler is seeking a Physical Therapist with 2+ years of experience for a 13-week home health/rehabilitation assignment with Tuesday–Saturday shifts.

Details

• 13-week contract in a home health and rehabilitation setting
• 8-hour shifts scheduled Tuesday through Saturday
• Treats patients of all ages, including those with developmental deficits, physical injury, or illness
• Responsibilities span direct patient care, plan of care development, and supervision of PTAs and Home Health Aides as applicable
• Documentation must comply with Medicare, State, Federal, JCAHO, insurance, and payer requirements

Requirements

• Minimum 2 years of physical therapy experience required
• Active state PT license required (license required if applicable)
• BLS/BCLS certification required
• Strong documentation skills with knowledge of Medicare and regulatory compliance standards
• DOB and SSN may be required for consideration

Additional Information

• Responsibilities include coordinating PT services, delivering individualized therapeutic care, and teaching rehabilitation skills to clients and caregivers
• Performs and monitors treatments using physical agents and equipment such as whirlpool baths, electrical muscle stimulation, and ultrasound
• Develops and revises individualized plans of care based on ongoing client assessments, physician orders, and discharge planning needs
• Supervises PTAs and Home Health Aides in accordance with applicable Federal and State regulations
• Participates in staff meetings, case conferences, and quality management/improvement programs
• Demonstrates leadership through communication, relationship building, and teamwork
