Description
American Traveler is hiring an experienced RN Case Manager for a home hospice position requiring prior home hospice case management experience and Epic proficiency.
Details
- Home hospice setting with a caseload of approximately 35–45 patients
- Day shift schedule, 5x8-hour shifts (10:00–18:00)
- 13-week contract
- Epic EMR required
Requirements
- Active VA or Compact RN license required
- Current BLS certification required
- Prior home hospice case management experience is required — no exceptions
- Proficiency with Epic EMR required
- Two professional references required for consideration
- Valid driver's license and Social Security card required for consideration
Additional Information
- Manage an active caseload of home hospice patients across the assigned territory
