Description

American Traveler is seeking a licensed Physical Therapist with Home Health experience for a 13-week travel contract in Anchorage, AK, requiring HCHB and OASIS proficiency.

Details

Home Health setting serving a geriatric patient population
Territory covers up to a 60-mile radius from the hub/office
Day shift schedule, 5x8 hours (8:00–17:00)
Caseload of 5–7 patients per day, with a minimum of 6 visits daily or 30 points weekly
Uses HCHB (HomeCare HomeBase) as the EMR
No on-call requirement
Local candidates are accepted at the travel rate
iPad provided for use in the field; mileage reimbursed at the IRS standard rate

Requirements

Current PT license required; license in hand required if applicable
Minimum 1 year of recent Home Health physical therapy experience required
OASIS experience required
Experience with HCHB EMR required
Current BLS certification required
Ability to manage a defined geographic territory of up to 60 miles from the home office

Additional Information

Timely communication and thorough documentation are essential in this role
Scrubs required in any color; jeans are not permitted
All time-off requests must be submitted at the time of initial consideration
2 professional references from a supervisor within the past 2 years required for consideration
Full 7-year work history with all gaps explained required for consideration
