Description
American Traveler is hiring a travel RN for a Case Management position at a large academic medical center, requiring active inpatient case management experience and proficiency with Epic.
Details
- Day shift position, 8:30 AM – 5:00 PM, five 8-hour shifts per week
- Inpatient case management unit within a large academic hospital (500+ bed facility)
- Adult Medicine/Surgery patient population
- No weekend, holiday, or on-call requirements
- Uses Epic EMR and post-acute referral platforms such as NaviHealth or Allscripts
Requirements
- Active RN license (Maryland or Compact) currently in hand
- Minimum 2 years of RN experience required
- Current, active inpatient case management experience required
- Experience at a comparable facility size (500+ licensed beds) required
- Current BLS certification required
- Proficiency with Epic EMR required
- Experience with post-acute facility referral platforms (e.g., NaviHealth, Allscripts) required
- SSN and DOB required for consideration
- 1 professional reference required for consideration
Additional Information
- Coordinates care for a caseload of adult patients from triage through discharge, including safe transition to home or post-acute settings such as nursing facilities, acute inpatient rehab, and local shelters
- Proactively communicates with payors to ensure approval of treatment plans and required resources
- Collaborates with the multidisciplinary team, patients, and families to support effective and efficient transition planning
- Maintains patient information in all departmental systems per operational guidelines
- Local candidates are accepted at the same rate as travel candidates
