Description
American Traveler is seeking an experienced RN Case Manager for an inpatient acute care hospital position requiring a CA RN license and a minimum of 5 years of RN Case Management experience.
Details
- Inpatient acute care hospital setting
- Case Management unit focused on care coordination and care transitions across the acute care continuum
- Collaborates with physicians, utilization managers, medical social workers, bedside RNs, interdisciplinary teams, third-party payers, and community resources
- May include assignment to the Emergency Department to address complex clinical and social situations to avoid unnecessary admissions
- Day shift, 5x8-hour shifts (8:00–16:30)
- Every other weekend required
Requirements
- Active CA RN license required
- Current BLS certification required
- Minimum 5 years of RN Case Management experience required
- Inpatient acute care hospital Case Management experience is required
- 2 supervisor references obtained within the past year required for consideration
- Copies of all current certifications required for consideration
Additional Information
- Responsible for care coordination and transitions planning, including developing comprehensive, cost-effective care plans in collaboration with the interdisciplinary team
- Performs utilization management duties including level-of-care reviews, length-of-stay monitoring, and documentation in the EHR to support LOC determinations
- Screens 30-day readmissions and proactively identifies barriers to care progression and transition
- Addresses complex clinical and social situations to prevent unnecessary admissions and ensure appropriate utilization of resources
- Identifies high-risk patients and makes referrals to community resources, palliative care, financial counselors, and other support services as appropriate
- Maintains current knowledge of CMS, Joint Commission Transitions of Care requirements, Conditions of Participation, and other applicable regulations
- Candidates must reside more than 50 miles from the facility to be eligible for travel pay rates
- Previous employees of this health system must have been separated for at least 1 year before returning as a traveler
- Only 1 holiday RTO request will be approved for this assignment
