Description
American Traveler is seeking a BSN-prepared RN for a Case Management position at an acute care, Level II Trauma facility in the Bronx, NY, requiring at least 1 year of inpatient case management experience and NY licensure.

- Acute care inpatient setting at a Level II Trauma facility in the Bronx, NY
- Case Management department focused on discharge planning for inpatient populations
- Primarily placed on Med-Surg or Telemetry floors following Internal Medicine and Family Medicine services
- Typical caseload ranges from 17–25 patients when paired with one Social Worker, or 12–17 patients on pilot floors where two Social Workers are present
- Utilization Review is handled by a separate department; this role is focused solely on discharge planning
- Charting performed in Allscripts EMR
- Monday through Friday, 8:00 AM – 4:30 PM (7.5-hour shifts with a 1-hour lunch break)
- Weekend and holiday coverage is occasionally required (rare, with manager approval); caseload and unit assignments may vary on those days
Requirements
- Active NY RN license required
- BSN required
- Current BLS certification required
- PRI (Patient Review Instrument) certification required — can be obtained online through the School of Albany (approximately 2 hours, ~$300)
- SCREEN Training certification required — can be obtained online through the School of Albany (approximately 2 hours, ~$300)
- Minimum 1 year of inpatient case management experience required
- Must be able to manage a caseload of at least 20 patients independently
- Strong critical thinking, time management, and interpersonal communication skills required
- One supervisor reference from within the last year and one peer reference from within the last 3 years are required for consideration
- Current Case Management skills checklist required for consideration
Additional Information
- Responsibilities center on discharge planning, including discharges to home, community, or SNF, as well as obtaining housing, shelter packets for homeless patients, hemodialysis arrangements, and KCI wound vac referrals
- Expected to round with physicians and the interdisciplinary team daily and collaborate closely with Social Workers on shared units
- Prior critical care experience is preferred
- Familiarity with CarePort, MCG, and/or InterQual criteria is preferred; some Utilization Review experience is a plus
- Orientation consists of 2 days of hospital orientation (including EMR training), followed by 1–2 weeks with a preceptor based on individual progress
- Business casual attire or scrubs in any color except royal blue or maroon (neutral colors preferred); lab coat provided
- A resource binder is provided to support clinical decision-making in the department
- Management is described as supportive, including encouraging staff to take full lunch breaks
