Description
American Traveler is seeking an experienced RN Case Manager with 5+ years of inpatient acute care case management experience and an active CA RN license for a 13-week travel contract in the San Francisco Bay Area.
Details
- Inpatient acute care hospital setting, Case Management unit
- Day shift schedule: 5x8-hour shifts, 8:00 AM – 4:30 PM
- Every other weekend required
- 13-week contract
Requirements
- Active CA RN license required
- Current BLS certification required
- Minimum 5 years of RN Case Management experience required
- Inpatient acute care Case Manager (hospital) experience is required — this is a firm requirement per the manager
- 2 supervisor references obtained within the past year required for consideration
- Candidates must reside more than 50 miles from the facility to qualify for travel pay rates
Additional Information
- Responsible for care coordination and care transitions planning throughout the acute care patient experience, collaborating with physicians, utilization managers, medical social workers, and bedside RNs
- Performs utilization management duties including level-of-care reviews, length-of-stay monitoring, and ensuring timely transitions to appropriate care settings
- Identifies and addresses complex clinical and social situations, psychosocial barriers, and high-utilizer patients with appropriate referrals and care plans
- Collaborates with an interdisciplinary team including financial counselors, Physician Advisors, third-party payers, and community resources
- Maintains documentation in the EHR and stays current with CMS, Joint Commission, and other regulatory requirements
- Former employees of this health system must have been gone for at least 1 year before returning as a traveler
- Only 1 holiday RTO request will be approved for the duration of the contract
