Description

American Traveler is seeking an experienced Home Health RN with a minimum of 2 years of experience and OASIS documentation proficiency for a 13-week travel contract in Vermont.

Details

• Home health setting serving a patient population with complex care needs including wound care, IV therapy, and symptom management
• Serves as Case Manager for assigned patients, managing caseloads and visit times in accordance with agency productivity standards
• High-tech skills required including IV use and maintenance, wound VAC, and complex wound care
• Uses McKesson for charting and documentation
• OASIS documentation is a core part of this role
• Schedule is Monday–Friday, 8am–4pm, with some on-call and weekend requirements

Requirements

• Active RN license in Vermont (VT) or a compact RN license that is currently valid and in hand
• Minimum 2 years of RN experience required
• OASIS documentation experience required
• Proficiency with McKesson EMR
• Skills in IV therapy, wound VAC, and complex wound care required

Additional Information

• Conducts comprehensive patient and caregiver assessments using the nursing process to develop and implement individualized plans of care
• Collaborates with interdisciplinary team (IDT) members including physicians, LPNs, physical therapists, social workers, and other providers to coordinate patient care
• Participates in IDT meetings, case conferences, and monthly peer review audits
• Performs patient and caregiver education on topics including medication side effects, treatment options, discharge instructions, and reportable conditions
• Responsible for accurate and timely completion of agency, state, and federal forms including HHABN and Medicare Notice of Non-Coverage
• Identifies and communicates interdisciplinary patient needs in a timely manner to ensure continuity of care
• Coordinates safe patient discharges and/or transfers, ensuring all required documentation is completed per policy
