Description

American Traveler is seeking a licensed Physical Therapist with at least 1 year of experience for a 13-week travel contract at a skilled nursing facility in Santa Rosa, CA.

Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid and currently active Physical Therapy license (license required if applicable)
• Minimum 1 year of physical therapy experience
• SNF or similar post-acute care setting experience preferred
• Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment
• Strong communication, interpersonal, and problem-solving skills
• DOB and SSN may be required for consideration

• Responsibilities include performing patient evaluations, developing individualized treatment plans, and providing therapeutic interventions to improve strength, mobility, and functional abilities
• Collaborate with an interdisciplinary team including occupational therapists, nurses, and physicians to coordinate and optimize resident care
• Patient and family education on therapeutic exercises, safe movement techniques, and home modifications is a key component of the role
• Accurate and timely documentation of assessments, progress notes, and treatment plans is required in accordance with facility and regulatory standards
