Description

American Traveler is seeking an experienced RN Case Manager with inpatient case management experience and an active NJ RN license for a hospital-based Care Management position.

Details

• Hospital-based inpatient Care Management unit
• Sub-acute patient population; experience with sub-acute patients required
• Inpatient case management, discharge planning, and utilization management responsibilities
• AM shift, 8:00 AM – 4:00 PM, Monday–Friday
• 37.5 hours per week, 7.5 hours per day
• Rotating weekend coverage every 4th or 5th weekend
• Holiday coverage required: 1 summer holiday and 1 winter holiday
• Floating to a nearby sister hospital may be required on an as-needed basis
• No on-call responsibilities
• EMR: Epic (experience required)

Requirements

• Active NJ RN license (NJ or Compact) required
• BSN required
• Minimum 2 years of acute care experience strongly preferred
• Inpatient case management experience required
• Experience with case management and managed care
• CCMC certification preferred
• Two professional references required for consideration

Additional Information

• Coordinates patients through the acute care hospitalization process
• Manages all aspects of discharge planning and care transition
• Performs utilization management and quality screenings
• Candidates must reside more than 50 miles from the facility; local candidates are not eligible
• Travelers may remain on assignment for up to one calendar year from their initial start date; a break of 30–60 days is required before returning to a new assignment
