Description

American Traveler is hiring a Home Health RN Float for a 13-week assignment requiring 1 year of certified home health care experience and 1 year of case management experience.

Details

• Home health setting covering North Region service areas including Cape Ann, Ipswich, and North Pod communities
• Float role providing care to home health patients across assigned cases in the region
• Day shift schedule from 08:00-16:30
• Weekend rotation required once every 6 weeks
• Ability to travel to patient homes as assigned
• Uses Epic for charting and documentation
• Uses UKG/Kronos for timekeeping

Requirements

• Active MA RN license required with no pending or conditional licenses accepted
• Both NURSYS and Board of Nursing verification required for license
• AHA-issued certifications only
• 1 year of Certified Home Health Care experience required (hospice, private care, or standalone case management experience does not qualify)
• 1 year of Case Management experience required
• Flu vaccine required with no religious or medical exemptions allowed
• 2 professional references required for consideration

Additional Information

• Responsibilities include assessing patient conditions, implementing care plans, and providing education and support to patients and families
• Perform admission processes and case management duties in accordance with organizational policies and standards of practice
• Deliver patient care within organizational philosophy and community health practice standards
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Local candidates residing within 60 miles of Boston, MA are accepted at a reduced rate compared to standard travel rate
• RTO must be disclosed at the time of consideration
