Description
American Traveler is seeking an experienced RN for a Care Manager/Case Management position in an acute care hospital setting, requiring an active NC or compact RN license and at least 2 years of RN experience.
Details
- Day shift schedule, 8:00am–5:00pm, 3 days per week (12-hour shifts)
- 36 hours per week
- Hospital-based Care Management unit within an academic medical center
- Covers a caseload across multiple specialty service lines including Cardiology, Nephrology, Trauma Surgery, Orthopedic Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown
- Patient population consists of acutely ill individuals with complex and varying medical needs
- No on-call, weekend, or holiday requirements
- Epic EMR used for assessment and care plan documentation
Requirements
- Active RN license in the state of North Carolina (NC or compact license) required
- Minimum of 2 years of RN experience required
- Hospital (acute care) case management, care management, or discharge planning experience required
- Nursing Diploma or ADN/ASN from an accredited nursing program required; BSN or MSN preferred
- Strong assessment and critical thinking skills required
- Epic EMR experience preferred
- Experience in academic or teaching hospital settings is highly beneficial
- Two professional references required for consideration
Additional Information
- Responsibilities include comprehensive patient assessment, care planning, and discharge planning to promote continuity of care and cost-effectiveness
- Participates in daily Care Management Touchpoints and weekly Complex Care Meetings (CCM) to monitor patient progression and address barriers to discharge
- Collaborates closely with an interdisciplinary team including Utilization Managers, Social Workers, and Care Management Assistants
- Coordinates family meetings, identifies post-acute service needs, and facilitates referrals to community resources and post-discharge providers
- Monitors observation patients throughout the day to ensure appropriate care progression
- Experience with care coordination and communication within interdisciplinary teams is strongly valued
