Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week travel contract covering Contra Costa County, CA.

Details

• Home health setting serving patients throughout Contra Costa County, CA, with potential territory expansion as needed
• Day shift schedule at 40 hours per week
• Productivity requirement of 30 points per week, with visit types including start of care, recertification, resumption of care, OASIS discharge, evaluations, and revisits
• Typical caseload averages 8 hours per day across a mix of visit types
• Documentation system: Kinnser (EMR)
• iPad and Verizon One Talk device issued for field use

Requirements

• Active CA Physical Therapist (PT) license required
• Minimum 1 year of home health experience required
• Current BLS certification required if applicable
• Ability to prioritize and manage a field-based caseload independently
• Strong communication skills and responsiveness during normal business hours

Additional Information

• Responsibilities include prioritizing start of care visits, supervising Physical Therapy Assistants (PTAs), and completing next-day mandatory reporting of start of care visits
• Clinical notes and all documentation must be submitted within 48 hours of each visit
• Timely communication with supervisor, intake, and office staff is expected
• Orientation consists of 3 full days: 1 day in-house and 2 days in the field
• Mileage reimbursement provided from first to last patient visit; parking and toll costs are also reimbursed
