Description
American Traveler is seeking a Home Health Occupational Therapist with at least 1 year of skilled home health experience and OASIS proficiency for a 13-week day shift contract serving the Hood River, OR area and surrounding communities.
Details
- Home health setting serving Hood River, OR and surrounding areas
- Day shift schedule, 5x8 hours (8:00–16:30); 4x10 and 3x12 schedules cannot be accommodated
- Caseload of up to 6 patients per day with a standard productivity expectation of 30 points
- Patient diagnoses include dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke
- Charting via Epic Remote Client (laptop) and Rover phone app; OASIS documentation required
- No weekend, on-call, or floating between like-unit requirements for this position; however, all travelers may be expected to float between facilities within the state
- Local candidates accepted; candidates residing within a 50–75 mile radius of the facility are considered local and are accepted at the same rate
- Daily travel radius of up to approximately 60 miles (60–90 minute total drive time); scheduling is optimized to minimize windshield time
Requirements
- Active OT license; OR and WA licenses required — license required if applicable
- Minimum 1 year of skilled home health experience
- OASIS experience required
- BLS certification required
- Epic EMR experience required
- Reliable personal transportation required
- 2 supervisory references covering at least 1 year of employment within the last 3 years in a comparable specialty or setting
- Full SSN and DOB required for consideration
- Active professional license at time of consideration
- First-time travelers with qualifying home health experience will be considered
Additional Information
- Provide occupational therapy services in patients' homes, including ADL training, functional activities, home assessments, adaptive equipment, and pre-discharge planning
- Preferred skills include prosthetics/orthotics, splinting (dynamic, functional, static), upper extremity prosthetics, lymphedema management, and myofascial release
- Preferred experience with national patient safety goals/core measures, billing, and OASIS documentation
- Business casual attire required; scrubs are permitted but no specific color is required and jeans are not allowed
- Candidates who have previously worked at this health system as permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility
- Minimal RTO is strongly preferred; limited time-off requests may be accommodated
- Previous work history at any facility within this health system (travel, per diem, or permanent) must be disclosed
