Description

American Traveler is hiring an experienced RN Case Manager for a 13-week travel assignment requiring a TN or Compact RN license and at least 1 year of case management experience.

• Position is in a Case Management unit within a hospital setting
• Day shift schedule: 8:00 AM – 4:30 PM
• 13-week contract

Requirements

• Active RN license — TN or Compact license required
• Minimum 1 year of case management experience
• Valid US Social Security Number and US home address required
• Must provide 2 professional references for consideration
• Copy of Driver's License required for consideration
• Full SSN and full DOB required for consideration
• Skills checklist required for consideration
• Copies of certifications and license verification/Nursys required for consideration

Additional Information

• Conduct comprehensive assessments of client needs, including medical, social, and financial factors
• Develop, implement, and update individualized care or service plans in coordination with interdisciplinary teams
• Coordinate services across providers, agencies, and community resources while advocating for client access to appropriate care
• Monitor client progress, adjust care plans as needed, and maintain accurate documentation in compliance with regulatory standards
• Educate clients and families on available resources, care plans, and facilitate communication among all care stakeholders
• Local candidates are accepted but may be subject to a reduced pay rate depending on proximity to the facility
