Description
American Traveler is hiring an RN Case Manager for a Discharge Planning role requiring an active MA RN license and at least 2 years of case management experience.
Details
- Case Management department in an acute care hospital setting
- Discharge planning focus within Case Management
- 40 hours per week, day shift Monday through Friday
- Schedule may consist of 4x10 or 5x8 shifts depending on operational need
- Shift hours are 07:30-18:00
- 13-week assignment
- Uses Epic EMR charting system
Requirements
- Active MA RN license required (no pending or conditional licenses accepted)
- Minimum 2 years of case management experience required
- At least 6 months of discharge planning case management experience in an acute care hospital required
- Experience in a large organization or teaching hospital with 500+ beds strongly preferred
- AHA-only certifications accepted
- Flu vaccine required with no religious or medical exemptions allowed
- Two professional references required for consideration

- Local candidates must reside within 60 miles of Boston, MA to be considered for the local rate
