Description

American Traveler is hiring a Case Management RN with 3-5 years of acute care experience for a day-shift assignment requiring BSN, CCM, and an active MA RN license.

Details

• Work in the Case Management department, primarily on an orthopedic unit within an acute hospital setting
• Day shift schedule, 8:00 AM to 4:30 PM (5x8 hours)
• Required to work every 3rd weekend
• Union facility with no special scheduling accommodations; must be able to work all 7 days as scheduled
• Epic charting system experience preferred
• Responsibilities include care coordination, discharge planning, needs assessment, and DME ordering

Requirements

• Active MA RN license required
• BSN required
• 3-5 years of nursing experience required
• Acute care experience required
• Medical-Surgical and Surgical Services specialty experience required
• BLS certification required
• CCM certification required for consideration
• Knowledge of CMS regulations and HIPAA guidelines
• 2 professional references required, with at least one from a Nurse Manager or higher

Additional Information

• Must be able to start as soon as possible
• Required to complete a Pass/Fail Tele test during orientation with an 80% passing score and only one attempt allowed; failure results in cancellation from the assignment
• Required to complete a Pass/Fail Medication exam at orientation
• Pre-start and orientation modules required, with additional annual modules
• Local candidates accepted at the same bill rate within a 75-mile radius
• Candidates must reside within 200 miles for consideration
• Returning staff must have a 12-month separation from the health system
• Up to 14 days of requested time off may be submitted for review, with no RTO allowed during the first week
