Description
American Traveler is hiring a Home Health RN Float for a 13-week assignment requiring 1 year of certified home care and case management experience with an active MA RN license.
Details
- Home health setting covering the North Region including Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End
- Float position requiring travel to patient homes as assigned across the coverage area
- 13-week assignment with a scheduled shift of 08:00-16:30
- Weekend rotation required once every 6 weeks
- Uses Epic for electronic charting and documentation
- Uses UKG/Kronos for timekeeping
Requirements
- Active MA RN license required (no conditional or pending licenses accepted)
- Both NURSYS and Board of Nursing verification required for licensure
- AHA-issued certifications only
- Minimum 1 year of certified home care experience required
- Minimum 1 year of case management experience required
- Ability to travel to patient homes as assigned
- Flu vaccine required with no religious or medical exemptions allowed
- Two professional references required for consideration
- Reliable transportation required to travel between patient locations
Additional Information
- Responsible for assessing patient conditions, implementing individualized care plans, and providing ongoing education and support to patients and families
- Perform admission processes and case management duties in accordance with organizational home care policies and standards of practice
- Deliver care in alignment with organizational philosophy, policy, and community health practice standards
- Maintain thorough and timely electronic documentation and interdisciplinary communication
- Local candidates may be considered; those residing within 60 miles of Boston will be classified as local
- RTO must be disclosed at the time of consideration
- Start date is contingent on credentialing clearance and may be rescheduled if not completed by the posted start date
- Parking is not validated in Boston locations; public transportation or paid public parking (approximately $17-$22 daily) may be required
