Description

American Traveler is hiring an RN Case Manager for an acute care hospital setting, requiring 2+ years of acute inpatient discharge planning experience and a BSN.

Details

• Acute care hospital Care Management department setting
• Coordinates patients through acute care hospitalization and manages discharge planning and transition processes
• Performs utilization management and quality screenings
• Handles discharge planning and placement in rehabilitation and subacute facilities
• AM shift schedule from 8:00 AM to 4:00 PM
• 37.5 hours per week over 5 days
• Rotating weekend requirement every 4th or 5th weekend
• Holiday coverage required for one summer and one winter holiday
• No on-call responsibilities
• May float to sister hospital location as needed
• Uses Epic EMR for charting

Requirements

• Active NJ or Compact RN license required
• BSN required
• Minimum 2 years of acute inpatient hospital discharge planning experience required
• Hospital experience with discharge planning and rehabilitation facility placement required
• Experience with subacute patients required
• Case Management and Managed Care experience required
• In-Patient Case Management experience required
• CCMC certification preferred

• Coordinates patient care throughout acute hospitalization from admission to discharge
• Manages all aspects of the discharge planning and transition process
• Conducts utilization management and quality screenings for assigned patients
