Description
American Traveler is hiring a Home Care RN for a 13-week hospice assignment requiring OASIS experience and a VT or compact RN license.
Details
- Home health and hospice setting serving the Southwest Region
- Case management for assigned patient caseload with productivity standards
- Schedule is Monday through Friday, 8am-4pm
- On-call and weekend rotation required
- Holiday coverage required
- Hospice coverage required while on call
- Uses McKesson documentation system for visit completion, lock-downs, and follow-up notes
Requirements
- Active RN license (VT or Compact) required upon acceptance
- Minimum 2 years of nursing experience required
- OASIS documentation experience required
- Experience with high-tech skills such as IV therapy, wound VAC, and complex wound care
- Working knowledge of evidence-based practice and State/Federal home health regulations
Additional Information
- Assess patient and caregiver needs and develop individualized plans of care
- Implement nursing interventions and medical plans of care to ensure safe, quality outcomes
- Collaborate with LPNs and interdisciplinary team members including physical therapy and social work
- Provide patient and caregiver education on medications, treatment options, and discharge instructions
- Complete required Agency, State, and Federal forms including HHABN and Medicare Notice of Non-Coverage
- Coordinate safe patient discharges and transfers with appropriate documentation
- Participate in monthly peer review audits and case conferences with the care team
- Communicate patient incident reports to the QI Specialist in a timely manner
