Description
American Traveler is hiring a Full-Time RN Case Manager with compact/CO RN licensure, requiring at least 3 years of hospital clinical experience and 5 years of case management experience.

- At least 3 years of clinical hospital nursing experience required
- At least 5 years of hospital case management experience required
- Current unrestricted Colorado RN license (compact accepted) required
- Current BLS certification required
- Certification in case management preferred, or willingness to obtain within 2 years of employment
Additional Information
- Develop, implement, and evaluate case management plans in collaboration with an interdisciplinary healthcare team
- Advocate for patients while coordinating resources, discharge planning, and utilization review processes
- Collaborate with payers and review organizations to ensure appropriate use of patient care resources
- Educate physicians and staff on appropriate levels of care and utilization issues
- Maintain current knowledge of case management, discharge planning, and nursing best practices through continuing education
- Document all patient case management contacts and information clearly and concisely
- Attend and actively participate in departmental and interdepartmental meetings related to Case Management and Social Services
