Description

American Traveler is hiring an experienced RN Care Manager for an acute care hospital setting requiring prior case management/discharge planning experience and North Carolina RN licensure.

Details

• Work in an acute care hospital Care Management department covering a 20-25 bed unit
• Patient population includes Cardiology, Nephrology, Trauma Surgery, Orthopedic Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown services
• Utilizes Epic EMR for documentation of assessments and care plans
• Schedule is Monday through Friday, 8:30 AM to 5:00 PM
• 40 hours per week, 8-hour shifts, 5 days per week
• No on-call, weekend, or holiday requirements

Requirements

• Active RN license in North Carolina (Standard or Compact)
• Minimum two years of Registered Nurse healthcare experience
• Prior experience as a case manager, care manager, or discharge planner in an acute care hospital setting required
• Nursing Diploma, ADN, or ASN from an accredited nursing school required
• BSN or MSN from an accredited nursing school preferred
• Strong assessment and critical thinking skills required
• Experience with acutely ill patients with complex medical needs

Additional Information

• Conduct comprehensive assessment, planning, implementation, and evaluation of individual patient needs to enhance quality of care and continuity
• Review and prioritize daily patient work list including new admissions
• Participate in daily Care Management Touchpoint meetings and CAPP (Communication and Patient Planning) meetings for assigned units
• Attend weekly Complex Care Meetings to collaborate on complex patient cases and identify trends
• Coordinate with Social Workers, Utilization Managers, and Care Management Assistants per departmental protocols
• Identify barriers to discharge and coordinate family meetings as needed to support care progression
• Educate patients and families on community resources and post-acute care options
• Monitor observation patients and determine discharge readiness in collaboration with the care team
• Identify required authorizations for post-discharge services and coordinate referrals
• Experience with educational or teaching hospital settings is helpful
