Description
American Traveler is hiring a Registered Nurse for a Home Health position in Honolulu requiring 1 year of home health experience and at least 3 months of OASIS documentation experience.
Details
- Home Health setting serving adult patients in their homes
- Day shift schedule, 5x8-hour shifts from 8:00 AM to 4:30 PM
- Productivity requirement of 25 units per week
- No on-call responsibilities
- Occasional floating to cover Kauai, Maui, and/or Big Island may be required, with float expenses paid
- 1:1 patient ratio
- HomeCare HomeBase (HCHB) charting system preferred
Requirements
- Active HI RN license required in-hand at time of consideration
- BLS certification (AHA) required
- Minimum 1 year of Home Health experience required
- At least 3 months of OASIS documentation experience required
- Skills required in adult home health care including admission assessment, case management, discharge planning, recertification, resumption of care, routine visits, and transfers
- Experience with blood glucose/glucometer management, colostomy/ileostomy care, G-tube and NG tube management, and nutritional assessment required
- Experience with wound care, pressure ulcer prevention/staging, drain management, and Wound V.A.C. management required
- Experience with medication and pain management, including use of pain scales, required
- Experience monitoring chemotherapy side effects and providing hospice pain management required
- Experience with port insertion/care and post AV fistula/shunt care required
- Experience with cast/brace/immobilizer device care and post-amputation/post-total joint replacement care required
- Experience with administration of blood/blood products, peripheral IV/central line/port management, IV starts, and phlebotomy required
- Skilled in clinical assessment documentation, service coordination, infection control precautions, obtaining vital signs, and oxygen safety/administration required
- Psych home health and tracheostomy care experience preferred
Additional Information
- Provide skilled home health nursing care to adult patients, including kupuna (elderly) populations, in the Honolulu community
- Perform comprehensive assessments, care coordination, and documentation using OASIS standards
- Scrubs (any color) and closed-toed shoes required
- Support patients across a range of clinical needs including wound care, catheter and tube management, chemotherapy monitoring, and pain management
- Opportunity to work within a collaborative home health care team supporting diverse patient needs across Oahu and neighboring islands
