Description

American Traveler is hiring an experienced RN Case Manager for an acute care hospital position requiring 2+ years of Acute Care Case Management/Discharge Planning experience and a CA RN license.

Details

• Acute care hospital setting within the Case Management department
• Day shift schedule (5x8 hours), 8:00 AM to 4:30 PM
• Patient ratios of 1:25 on weekdays and 1:45 on weekends
• Every other weekend work required to meet department needs
• No floating expected to other units
• Epic EMR experience required

Requirements

• Active CA RN license required (pending license accepted if licensed by start date)
• BLS certification required
• Minimum 2 years of Acute Care Case Management/Discharge Planning experience required
• Prior hospital experience required; behavioral health experience does not qualify
• Experience with care coordination, discharge planning, disease management, needs assessment, DME ordering, plan of care, and prior authorizations
• Familiarity with CMS, Department of Health, DRG, and Joint Commission/Core Measures/National Safety Goals regulatory standards
• Must be a seasoned traveler; first-time travelers will not be considered
• Two supervisory references required, covering 1 year of employment within the last 3 years

Additional Information

• Coordinate patient care including admission criteria review, benefits eligibility, and discharge planning
• Conduct needs assessments and order durable medical equipment as needed
• Manage prior authorizations and ensure compliance with regulatory standards
• Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• RTO requests limited to no more than 7 days and must be submitted upfront
• Business attire or scrubs accepted as dress code
• Candidates must reside within 50 miles for local rate consideration
• Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the facility
• Travelers may be expected to float between sister facilities near the contracted location
