Description
American Traveler is seeking an experienced RN OB Case Manager for an outpatient specialty clinic setting, requiring an active AK RN license and prior OB case management experience.
Details
- Ambulatory OB/GYN specialty clinic setting
- Provides outpatient case management and nursing care across the continuum of care
- Monday through Friday day shift schedule
- 15-week assignment length
- May float to other departments or assignments as needed
Requirements
- Active, unrestricted Alaska RN license required at time of submission
- Minimum 2 years of nursing experience required
- OB case management experience required
- BLS certification required
- Case management certification preferred
- Graduate of an accredited nursing program required, Bachelor's degree preferred
- High school diploma or equivalent required
Additional Information
- Performs comprehensive patient assessments and develops individualized plans of care
- Coordinates with providers and interdisciplinary teams to facilitate referrals, follow-up care, and continuity of services
- Provides patient and family education related to care, medications, and discharge needs
- Documents assessments, interventions, and outcomes accurately in the electronic health record
- Advocates for patients and assists them in navigating the healthcare system
- Participates in quality and performance improvement activities
- Provides guidance and delegation to ancillary staff and supports orientation and mentoring of staff, students, and peers
- Must live at least 100+ miles from the facility to be eligible for consideration
- Housing and transportation are not provided and are the candidate's responsibility
- Start date will be no less than 5 weeks after fingerprint cards are received for processing
