Description

American Traveler is seeking an experienced RN OB Case Manager for an outpatient specialty clinic setting, requiring an active AK RN license and prior OB case management experience.

Details

• Ambulatory OB/GYN specialty clinic setting
• Provides outpatient case management and nursing care across the continuum of care
• Monday through Friday day shift schedule
• 15-week assignment length
• May float to other departments or assignments as needed

Requirements

• Active, unrestricted Alaska RN license required at time of submission
• Minimum 2 years of nursing experience required
• OB case management experience required
• BLS certification required
• Case management certification preferred
• Graduate of an accredited nursing program required, Bachelor's degree preferred
• High school diploma or equivalent required

Additional Information

• Performs comprehensive patient assessments and develops individualized plans of care
• Coordinates with providers and interdisciplinary teams to facilitate referrals, follow-up care, and continuity of services
• Provides patient and family education related to care, medications, and discharge needs
• Documents assessments, interventions, and outcomes accurately in the electronic health record
• Advocates for patients and assists them in navigating the healthcare system
• Participates in quality and performance improvement activities
• Provides guidance and delegation to ancillary staff and supports orientation and mentoring of staff, students, and peers
• Must live at least 100+ miles from the facility to be eligible for consideration
• Housing and transportation are not provided and are the candidate's responsibility
• Start date will be no less than 5 weeks after fingerprint cards are received for processing
