Description

American Traveler is hiring a Case Management RN for a 13-week contract requiring 2 years of acute case management experience and a TX or compact RN license.

Details

• Work in the Case Management department of an acute care hospital setting
• Provide case management services for both inpatient and observation patients
• Patient ratio of approximately 25:1 for case managers
• Uses MIDAS, CarePort, and InterQual systems
• Charts using the Meditech EMR
• Day shift schedule from 8:00 AM to 4:30 PM
• 5 eight-hour shifts per week with rotating weekends
• No on-call requirements

Requirements

• Active TX or compact RN license
• 2 years of recent acute care case management experience required
• No gaps in work history of 90 days or more within the past 12 months

Additional Information

• Perform comprehensive assessments of psychosocial, medical, and discharge needs for patients and families
• Coordinate the plan of care and drive discharge planning by collaborating with the multidisciplinary team
• Evaluate progression of care using InterQual throughout the episode of care
• Business casual dress code
• Must reside outside a 50-mile radius of the facility to be considered for this role
