Description

American Traveler is seeking a Home Health Physical Therapist with 2+ years of experience and Oasis proficiency for a 13-week assignment providing in-home care.

Details

• Home health setting covering PT, OT, and Speech services across Marian and Arroyo Grande areas
• Patients seen in their homes within an assigned geographical region
• Uses Cerner as the EMR
• Typical schedule is Monday-Friday, 8am-5pm, with flexible shift timing
• 8-hour day shifts, 40 hours per week
• Works alongside PTA/OTA rehab aides, typically permanent staff
• Requires working every 3rd weekend
• Minor holiday coverage expected; major holidays only if needed

Requirements

• Active PT license required for state of practice (license required if applicable)
• BLS certification required
• Minimum 2 years of experience required
• At least 1 year of experience in PT, OT, or Speech required
• Experience with Oasis (government-mandated home health form) required
• Home health setting experience preferred but not required
• Must provide a copy of driver's license for consideration
• Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, including employment dates and rehire eligibility

Additional Information

• Treats patients with diagnoses including orthopedic conditions, strokes, joint issues, and spinal injuries
• Speech therapy caseload may include a variety of diagnoses
• Business attire with a white lab coat required; no jeans
• Floor-specific orientation lasts 1.5-2 weeks
• Mileage is not reimbursed separately as this is an all-inclusive bill rate
• Candidates may not have been directly employed by any CommonSpirit, CHI, or Dignity Health facility within the past year, though PRN work through outside staffing firms is acceptable
