Description
American Traveler is seeking an RN Care Manager for a Home Health position requiring a CA RN license and EPIC experience.
Details
- Home health setting providing skilled nursing care to patients in their homes
- Manages an assigned patient caseload independently while collaborating with a multidisciplinary care team
- Day shift schedule from 8:00 AM to 4:30 PM
- EPIC EMR experience required
Requirements
- Active RN license in California required
- Current BLS/CPR certification from an approved vendor required
- Valid driver's license required, must be 21 years or older
- Current automobile insurance and reliable personal vehicle required
- BSN preferred
- Prior acute care facility or home care experience preferred
- Minimum of 2 years of experience in the specialty required
Additional Information
- Completes initial and ongoing comprehensive patient assessments and updates plans of care as needed
- Provides direct and indirect skilled nursing care and coordinates services with other disciplines
- Educates and counsels patients and families on healthcare and nursing needs
- Documents patient care delivery, status, and response to treatment in accordance with policy
- Accepts local candidates at the same rate as travel candidates with no radius restriction
- Candidates with more than 5 days of requested time off will not be considered
- Candidates with any prior adverse licensure action will not be considered
