Description

American Traveler is hiring an experienced RN Care Manager for an acute care hospital setting, requiring case management or care management experience and a NC or compact RN license.

Details

• Work within a 20-25 bed unit covering Cardiology, Nephrology, Trauma Surgery, Ortho Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown services
• Day shift schedule from 8:00am to 5:00pm
• 36 hours per week over 3 days per week
• No on-call requirements
• No weekend requirements
• No holiday requirements
• Uses Epic EMR for documentation and care planning

Requirements

• Active RN license (NC or compact) required
• 2+ years of Case Management or Care Management experience in an acute care setting required
• 2+ years of healthcare experience as a Registered Nurse required
• Nursing Diploma, ADN, or ASN from an accredited nursing school required; BSN or MSN preferred
• Strong assessment and critical thinking skills required

Additional Information

• Provide comprehensive assessment, planning, implementation, and evaluation of individual patient care needs
• Coordinate discharge planning and care progression for patients with complex medical conditions
• Participate in daily Care Management Touchpoints and CAPP meetings to review patient progression and discharge readiness
• Attend weekly Complex Care Meetings to collaborate on complex patient cases and identify trends
• Consult with Social Work and Utilization Management per departmental protocols
• Coordinate referrals to community resources and post-acute care providers
• Communicate with patients and families regarding medical milestones and discharge plans
• Experience in an educational or teaching hospital setting is beneficial
