Description

American Traveler is hiring an RN Case Manager for a discharge planning role in an acute care hospital, requiring 2+ years of case management experience and an active MA RN license.

Details

• Case Management department position focused on discharge planning
• Day shift schedule, Monday through Friday, 40 hours per week (may be structured as 4x10s or 5x8s depending on operational need)
• Shift hours 07:30-18:00
• 13-week assignment length
• Uses Epic for charting

Requirements

• Active MA RN license required (no pending or conditional licenses accepted)
• Primary Source Verification of license required within 30 days of start, updated upon expiration (NURSYS and BON required)
• AHA certifications only
• Minimum 2 years of case management experience required
• At least 6 months of discharge planning case management experience in an acute care hospital required
• Experience in a large organization or teaching hospital with 500+ beds strongly preferred
• Flu vaccine required with no religious or medical exemptions allowed

Additional Information

• Responsible for coordinating discharge planning for patients in an acute care hospital setting
• Local candidates accepted; permanent residence within 60 miles of Boston qualifies for local candidate rate
• Uses UKG/Kronos for timekeeping
• Parking not validated at Boston locations; public transportation or paid public parking required
